Purpose
The purpose of this Request for Proposal (RFP) # MEDIOMC26023 is to solicit applications that will enable the Iowa Department of Health and Human Services (referred to as Agency) to select the most qualified healthcare applicants to serve as partners with rural Academic Institutions (referred to as schools) to provide medical, mental/behavioral, and dental health services,utilizing schools as the site of service. The eligible applicants will bring together medical, mental/behavioral, and/or dental health service delivery entities to collaboratively provide health care services at school locations. The purpose of school-based services is to expand and sustain access to care in rural communities, foster new partnerships among local providers, and improve health outcomes by embedding health services in schools.
