Purpose:
The purpose of this RFA is to provide guidance for current contractors to submit continuation applications to the Iowa Department of Health and Human Services (referred to as Agency) to renew service provisions as described by and within the project period established by the RFP 58823021. Services covered by this application include 1st Five Healthy Mental Development (1st Five) services.
The basics of 1st Five include:
A. Engaging, training, and providing technical assistance to primary care providers on the 1st Five program and screening and surveillance tools for children birth to age five

Targeting the population of children birth to age five (up to a child’s fifth birthday)

Increasing use of surveillance and standardized developmental screening in primary care

Providing a one-step referral resource for primary care providers

Connecting referred children (and their parents/caregivers) to existing services in their local communities

Keeping primary care providers informed about children’s progress
B. Supporting healthy social, emotional and cognitive development for children birth to age five.
The Agency is actively working to strengthen our capacity to address health inequities in Iowa. Health equity is defined as supporting opportunities for everyone to live the healthiest life possible through the reduction of barriers in environmental, social, economic and other conditions which impact health. 1st Five promotes health equity through increased use of standardized developmental screening tools on a universal basis to minimize bias in assessment and by individualizing the connection to services based on a child’s and their parents’/caregivers’ unique needs for support. The standardized developmental screening tools supported are those that have been tested across disparate population groups and shown to reliably indicate concerns. Connection to services addresses both medical and social determinants of health.Schedule of Important Dates (All times and dates listed are local Iowa time.)

Expansion into unfunded counties within CSA 1 will only occur at the direction of the Department and if funding becomes available in the future. |
