  Purpose  

  The purpose of this Request for Proposal (RFP) #PHTHOCC26004 is to solicit applications that will enable the Iowa Department of Health and Human Services (referred to as Agency) to select the most qualified applicant to develop and implement evidence-based strategies to track, monitor, and address patient clinical and social services and supports needs, implement the principles of team-based care, and link community resources with clinical services to mitigate social support barriers to improve outcomes for patients at the highest risk of cardiovascular disease with a focus on hypertension and high cholesterol in the health system.
