    Purpose:

    The purpose of this Request for Proposal (RFP) # 58825018 is to solicit applications that will enable the Iowa Department of Health and Human Services (referred to as Agency) to select the most qualified applicant to provide Brain Injury Resource Facilitation and Training. Brain Injury Resource Facilitation (BIRF) is an ongoing support provided to individuals with brain injury on coping with issues of living with a brain injury and assisting such individuals in transitioning back to employment and living in the community. BIRF is a person-centered program intended to provide a linkage to existing services, increase the capacity of Iowa’s providers of services to persons with brain injury, and to conduct community outreach, awareness and education. BIRF enhances the usage of support commonly available to an individual with brain injury from the community, family and personal contacts; and links individuals served to appropriate services and community resources, while providing support needed for accessing, securing and maximizing the private and public funding available. BIRF provides brain injury-specific information, support and resources; as well as provide training and consultation on providing appropriate brain injury services and support.

Oct 9, 2024 11:06 AMClarification on Outcome Measures and Success Criteria: Are there specific performance measures beyond those mentioned in the RFP (e.g., client satisfaction, number of individuals served, measured changes in individual or population social determinants of health)? Contract performance measures are described in RFP Section 2.03E Contract Performance Measures. It includes submission of Semi-Annual Progress and Data Reports and Final Report by the dates established in RFP Section 2.03B Deliverables. Section 2.03B describes details of these reports.

Within the Project Work Plan component in Iowagrants.gov, Applicants will develop SMART (Specific, Measurable, Attainable, Realistic, and Time-sensitive) objectives for each goal and describe the rationale/measure for the objective. There may be multiple objectives for a specific goal.

Oct 9, 2024 11:33 AMBudget and Funding Clarifications (Section 1.05 and 1.6 of Document A and Section 1.6.2 of Document C): Is the 15% administrative cap inclusive of all overhead and general operating expenses other than equipment? scope of services or deliverables? Per RFP Section 2.02 Definitions, “administrative costs” means those costs and fees that may include, but are not limited to, those that have been incurred for common or joint purposes or objectives, benefitting more than one cost objective, and/or cannot be readily identified with a particular final cost objective. For the purposes of this RFP, examples of Administrative Costs shall include general administration and general expenses such as accounting, expenses of executive officers, personnel administration, costs of operating and maintaining the facility, rent and lease payments, utilities, data collection and data processing costs, printing, communications equipment and services, depreciation, and other costs necessary to support the delivery of services. Per Section 2.04A1(1b) Administrative Cost Charges, Applicants may charge Administrative costs, only in the absence of a federally approved Negotiated Indirect Cost Rate Agreement or an Indirect Cost Plan recognized by a state cognizant agency (local governments). Administrative Costs are capped at (limited to) 15% of the direct costs proposed in the budget.

Scope of Work and Deliverables outlined in the RFP and resulting contract will not change. On an annual basis, the Contractor will submit a work plan and line-item budget adequate to support the proposed work for Agency approval as part of the Contractor’s annual renewal/extension and based on the award amount entered by the Agency within Iowagrants.gov

Applicants may utilize the Title/Position Description and/or Roles and Responsibilities section to identify individuals who function as the specific project personnel required, as outlined in RFP Section 2.03C Contractor’s Personnel for Project Implementation.

As described in RFP Section 3.02 Application Forms, Applicants must complete each application form listed, following the instructions within that section and within Iowagrants.gov. The Business Organization component requires information about the applicant organization, including business structure. The Background and Demonstrated Experience component requires information about the applicants demonstrated experience in provision of services and established community partnerships. Applicants will include relevant information about their background and experience that has prepared them for the work as described in this RFP, including community partnerships and services the applicant has already developed and established.

Per RFP Section 1.03, eligible applicants must be nonprofit organizations with staff capacity to provide statewide Brain Injury Resource Facilitation and training via virtual and in person sessions (method of delivery will be determined by the Agency). Per Section 2.03 Scope of Work Goal 1, Brain Injury Resource Facilitation is offered statewide and can take place via phone, email, text or in person. Per Goal 2, Applicants shall describe specific activities that will be completed to ensure training and education services are provided, which includes in person and virtual sessions.

July 1, 2025 - June 30, 2026. (See section 1.05: “Available funds” “Applicants will be expected to submit budgets for the first six-month contract period (Contract Year 1) and first annual contract period (Contract Year 2). Refer to section 2 and 3 of this RFP.” Subsequent question/clarification: a) Is a six month line item budget being requested only? b) Are both a six month and a subsequent 12-month budget requested? c) If both (two budgets), where in the online platform are more than one line-item budget to be submitted as it appears there is a framework for only 1 budget. A six-month budget is requested for the time period of January 1, 2025 – June 30, 2025 (Year 1). Each subsequent contract year, the selected contractor will submit a budget at least 90 days prior to the beginning of the contract year.
