May 22, 2024 8:51 AMWill the funded partner be responsible for initiating a partnership with the CAH's or will the hospitals be made aware of the program through HHS? Will hospitals be required to participate in the program and work with the funded partner? Or can they opt out? Per the RFP 2.01 Background, “This RFP covers the Population Health Flex Program Area, which is an optional program area funded by the Federal Office of Rural Health Policy (FORHP).” Critical Access Hospitals are not required to participate. Per the RFP 2.03 Scope of Work (Objective 2: (the funded contractor) will, “Provide population health education and training),” for CAHs.
