    Purpose

    The purpose of this Request for Proposal (RFP) # 58825005 is to solicit applications that will enable the Iowa Department of Health and Human Services (referred to as Agency) to select the most qualified applicant to support Iowa’s eighty-two critical access hospitals (CAHs) in population health by building capacity among rural hospitals to identify and address needs, improving health outcomes for rural communities, and ensuring that patient care is systematically integrated throughout the rural health care delivery system.

    The Agency is actively working to strengthen our capacity to address health inequities in Iowa. Health equity is defined as the attainment of the highest possible level of health for all people by achieving the environmental, social, economic and other conditions in which all people have the opportunity to attain their highest possible level of health. This program promotes health equity by improving the quality of care delivered by CAHs and strengthening the healthcare infrastructure, thereby working to ensure access to quality health care, especially in rural and underserved areas of Iowa.

May 22, 2024 8:51 AMWill the funded partner be responsible for initiating a partnership with the CAH's or will the hospitals be made aware of the program through HHS? Will hospitals be required to participate in the program and work with the funded partner? Or can they opt out? Per the RFP 2.01 Background, “This RFP covers the Population Health Flex Program Area, which is an optional program area funded by the Federal Office of Rural Health Policy (FORHP).” Critical Access Hospitals are not required to participate. Per the RFP 2.03 Scope of Work (Objective 2: (the funded contractor) will, “Provide population health education and training),” for CAHs. The funded contractor will be introduced by the Flex Coordinator to CAHs, after which time the contractor would begin the RFP 2.03 Scope of Work.
