    Purpose

    The purpose of this RFA is to provide guidance for current contractors to submit continuation applications to Iowa Health and Human Services (referred to as Agency) to renew service provisions as described by and within the project period established by the RFPs #58823005, #58823026 and #58823028. Services covered by this application include services at the community level for Child & Adolescent Health (CAH) services. CAH services include Hawki Outreach, Early ACCESS, Healthy Child Care Iowa (HCCI), and I-Smile™. The I-Smile™ program also includes I-Smile™ @ School. The CAH program has identified eight priority populations that have experienced less access to child and adolescent health care and experience health inequities as a result. Specific inclusion of these priority populations in planning, implementation, and evaluation of CAH programming is necessary to address these health inequities. Priority populations may be eligible for and/or require additional services, outreach, and accommodation to assure access to services and increase health equity in Iowa. The priority populations for the CAH program are:

    1. African American, Black, or African
    2. Asian, Pacific Islander
    3. Fathers
    4. Hispanic or Latinx 
    5. Lesbian, gay, bisexual, transgender, queer, intersex (and emerging terms) (LGBTQI+)
    6. Native American, Alaska Native
    7. Persons with disabilities
    8. Refugees or immigrants

Apr 26, 2024 11:22 AMThere are two Hawki Activity workplan forms. One is a tab within the CAH workplan excel and the other is an excel on its own under the Hawki Outreach activity work plans component. There appears to be just a little formatting difference between the two but otherwise the same. Which one are we to complete? Please complete only the Hawki tab in the CAH Activity Work Plan to document your activities. There is no need to complete the Hawki Work Plan.
