Purpose
The purpose of this RFA is to provide guidance for current contractors to submit continuation applications to the Iowa Department of Health and Human Services (referred to as Agency) to renew service provisions as described by and within the project period established by the RFP #58822024. Services covered by this application are for Centers of Excellence that encourage innovation collaboration among regional health care providers in rural areas.
