Purpose
The purpose of this Request for Proposal (RFP) # 58825006 is to solicit applications that will enable the Iowa Department of Health and Human Services (referred to as Agency) to select the most qualified applicant to implement nutrition and physical activity policy, systems and environmental change community-based strategies for low-income Iowans.
The Agency is actively working to strengthen our capacity to address health inequities in Iowa. Health equity is defined as supporting opportunities for everyone to live the healthiest life possible through the reduction of barriers in environmental, social, economic and other conditions which impact health. This program promotes health equity by focusing on populations that experience health disparities. Specifically, the program is aimed at individuals with low-incomes in urban and rural communities.Schedule of Important Dates (All times and dates listed are local Iowa time.)

Bike racks, bus shelter accessible seating. Exercise equipment for evidence-based programs for seniors such as exercise mats, free weights, pool supplies. Training costs and curriculum for evidence-based exercise programs. Supplies for creating a storywalk, sensory path and wayfinding signs. Items for community gardens such as soil, cages, lumber, hoses, plants. What if the project includes multiple locations? | No, the Site Eligibility Form should be submitted with the application. The site address should be the specific location where the project will take place. Every site where you will be doing work will need to be a qualified site or have a clear connection to how the site is serving or reaching eligible audiences. Complete one site eligibility form for each site. |
