    Purpose 

    The purpose of this Request for Proposal (RFP) # 58824024 is to solicit applications that will enable the Iowa Department of Health and Human Services (referred to as Agency) to select the most qualified applicant to provide peer to peer practice consultation with primary care providers (PCP) in Iowa to increase access and quality of adolescent well visits through long-term practice changes. 

    The Agency is actively working to strengthen our capacity to address health inequities in Iowa. Health equity is defined as supporting opportunities for everyone to live the healthiest life possible through the reduction of barriers in environmental, social, economic and other conditions which impact health.

Or is an individual required?A specific practice consultant (individual) must be named in the application. The specific guidance regarding this is: RFP p 26. “Practice Consultant” means a specific, named primary care provider with a minimum of 2 years experience training and/or consulting with other primary care providers, and a minimum of 2 years experience providing primary care, including comprehensive well visits, to adolescents within the last 5 years. This position may be an employee or subcontractor. RFP p 38 Practice consultant(s) must be identified by name, they can be employees or subcontracted. To be hired/to be named/to be determined/to be contracted with or naming a subcontract organization but not a specific practice consultant does not meet criteria. See the Draft Application Score Tool for Criteria that will be used by the Review Committee to evaluate and score this form. E. FY 24 RFP Adolescent Well Visit Scoring Tool (Draft) Practice Consultant Background and Demonstrated Experience Form Each practice consultant must be named in the application, To Be Named/To Be Hired/To Be Determined/To Be Subcontracted does not meet criteria. Listing an organization as the employer or subcontractor without specifically naming the practice consultant does not meet criteria (e.g. We will utilize one of the 10 pediatricians meeting all criteria from our Pediatric Department. Or Benson Pediatric Group employs 4 pediatricians who all meet criteria to be a practice consultant, we will subcontract with one of them. Neither example meet criteria). Practice consultant may be an employee or a subcontract.
