  Excerpts from the Community Based Program - Maternal Health:

  Purpose

  The purpose of this RFA is to provide guidance for current contractors to submit continuation applications to the Iowa Department of Public Health (referred to as Agency) to renew service provisions as described by and within the project period established by the RFP #58823004 and #58823024. The Agency’s Title V Maternal Health Programs provide support and education for pregnant women and their families in all Iowa counties based on the individualized needs of the clients and community as outlined in IAC 641—76 Maternal and Child Health Program. These services include, but are not limited to: community education sessions, community coalition building activities, linking pregnant women to medical and dental homes, providing presumptive eligibility services for Iowa Medicaid, and Title V Maternal Health Direct Care Services.
