Purpose

The purpose of this Request for Proposal (RFP) # 58822024 is to solicit applications that will enable the Iowa Department of Public Health (referred to as Department) to select the most qualified applicant to create Centers of Excellence that encourage innovation and collaboration among regional health care providers in rural areas. Centers of Excellence are innovative care models that specialize in specific areas of medicine, offer a variety of services and treatments, and create regional access to specialty care with a focus on creating access to specialty care in rural areas where access is currently lacking.

09/15/2021 This opportunity is for a project period starting December 15, 2021 through June 30, 2027. The first contract period lasts through June 30, 2022 and has a determined funding amount. While we understand that future funding is within the discretion of the Department, can you provide any guidance related to funding level in out years as this will impact the scope of our project. The source of funding for RFP 58822024 is state fiscal year 2021 general fund appropriations. Funding availability for subsequent years within the project period of December 15, 2021 through June 30, 2027 will be determined on a yearly fiscal year basis based upon future legislation and general fund appropriations. Applicants should plan for the scope of their project with this understanding.
09/23/2021 In our initial phase we will be doing a proof of concept in a subset of rural counties. The intent would be to assess and grow the program over the next five years to include future counties based upon data. For the coversheet section should the congressional and legislative county and district selections reflect the first-year counties only? The congressional and legislative counties and district selections on the cover sheet should only reflect first-year counties.
09/23/2021 Does the state anticipate that budgets in Years 2-3-4-5 will have fixed deliverables? If so, will they comprise a similar amount - more than 50% of the budget? Any detail related to portion of the budget that will be determined by the applicant would be appreciated. Funding availability for subsequent years within the project period will be determined on a yearly state fiscal year basis (July 1st - June 30th). It is anticipated that if funding is available for subsequent years, at a minimum a “Final Report” will be included as a fixed deliverable. Budget amounts for fixed deliverables have not been determined and will be based upon future legislation and general fund appropriations.
09/23/2021 While we do not anticipate any contracted services in the first performance year, will applicants be able to add these services in out years of the grant? Subcontracts may be utilized in future contract years.
09/23/2021 Does the state anticipate requesting greater funding in Years 2-3-4-5 of the budget to correspondence to implementation activities as opposed to planning activities? While we understand that the budget is subject to an annual appropriation, we are requesting to understand the state's intention related to its ask. The current funding level was for approximately a 6-month period. Is it fair to assume that funding levels will be at minimum be doubled? Funding availability for subsequent years within the project period will be determined on a yearly state fiscal year basis (July 1st - June 30th) and will be based upon future legislation and general fund appropriations. It is currently unknown at this time what the funding amount will be for future years.
