Purpose
The purpose of this Request for Proposal (RFP) # 58822002 is to solicit applications that will enable the Iowa Department of Public Health (referred to as Department) to select the most qualified applicant to provide education on nutrition and physical activity for older adults, age 60 and above, that are eligible for the Supplemental Nutrition Assistance Program - SNAP.

If writing for more than one county, are we required to have 3 meetings per month in each county or 3 meeting total for the service area? |
According to section 2.02 Description of Work and Services under Minimum Program Requirements, a minimum of three meetings every month that include one live virtual meeting, one conference call and one in-person opportunity (as allowed by current federal and state public health guidance or agency policy) in the service area. Additionally, according to section 2.05 Work Plan and Technology Requirements under Work Plan, although the minimum is three meetings per month per service delivery area, the applicant may need to provide meetings at multiple locations, with multiple modes, on multiple dates, at multiple times to better serve participants. It is recommended that groups are limited to 10-15 participants. |

Per the RFA, "applicants will identify the proposed service
area for delivering the Fresh Conversations program (counties in the service area should be contiguous)."
If the agency that is applying has a statewide reach, is this still required? For example, if we have facilitators who would be able to serve 20 counties, would these 20 counties have to be contiguous?

According to section 1.04 Service Delivery Area, applicants may apply for one or more counties. If applying for multiple counties, the counties must be contiguous. |

What is the difference between the “table of organization” requested at the bottom of the Fresh Conversations Project Personnel Form and the Business Organization Form—Table of Organization section that is required in the Business Organization section? |
RFP section 2.03 Staffing or Personnel Requirements, the table of organization requested refers to the structure of the organization for the Fresh Conversations Program. RFP section 3.02 under Business Organization, the requested table of organization refers to the organization structure of the applicant’s business. |

Can the Nutrition Education Coordinator and Facilitator be the same person, even if there are others facilitating eventually? |
Yes, the Nutrition Education Coordinator and Facilitator can be the same person. Per RFP section 2.03 Staffing or Personnel Requirements, the Nutrition Education Coordinator may also host meetings. RFP section 3.02 Application Forms under Fresh Conversations Project Personnel, under Nutrition Facilitator, name and email address can be listed as “vacant” if person is unknown at this time. |

We currently subcontract with our Fresh Conversations Coordinator but she will become a full time employee of our agency in May 2021. For application purposes, do we list her as a subcontractor or employee? |
RFP section 1.02 Project Period and Contract Term, the project period is October 1, 2021- September 30, 2023. The applicant should only submit a Subcontract Plan if a subcontractor will be used for this funding opportunity during the project period. |
