It is the responsibility of the Applicant to review all attachments listed in the section below, including the Request for Bid (RFB).

The issuance of the RFB in no way constitutes a committment by IDPH to award a contract.

  1. INTRODUCTION

    The purpose of this Request For Bids (RFB) is to solicit bid applications which will enable the Iowa Department of Public Health, hereafter known as the Department, to select the most qualified Applicant to provide the development, promotion, and implementation of injury prevention and education initiatives for children (age birth to 21).

     

    Project and Contract Period:

The project period shall be from August 16, 2018 to June 30, 2019.

 

The Department expects the initial contract period to be an 10 and 1/2 month term from August 16, 2018 to June 30, 2019. The Department shall have the option to renew the contract for up to one additional one (1) year term subject to review of the continuation application, contractor performance and compliance with the terms and conditions of the contract, availability of funds, program modifications, or any other grounds determined by the Department to be in the Department’s best interests.  The scope of services must be completed within the project period August 16, 2018 to June 30, 2019.

 

Available Funds

 

Applicants may apply for up to $2,000 for the total August 16, 2018 to June 30, 2019 period.  Actual total awards and individual contract funding levels may vary from that listed or funding may be withdrawn completely, depending on availability of funding or any other grounds determined by the Department to be in the Department’s best interests.

 

The issuance of this RFB in no way constitutes a commitment by the Department to award a contract.

 

SCOPE OF SERVICES

 

The Bid Application shall include the Applicant’s plan for accomplishing the work set forth below and should include sufficient information regarding the Applicant’s ability to perform the services. Sufficient information shall be provided to enable the Department to make a judgment about the Applicant’s ability to perform the work identified in the Scope of Services.

The goal of the program is to develop, promote, and implement injury prevention and education initiatives for children (age birth to 21).

 

Injury prevention activities that may be considered for funding include but are not limited to:

 

  • Education and training materials;
  • Equipment, supplies, and materials that may include child passenger safety equipment (child passenger safety equipment must be distributed by a nationally certified child passenger safety technician);
  • Public information and education campaigns;
  • Conference/seminars/workshops;
  • Contractual services;
  • Personnel costs

 

 

III. ADMINISTRATIVE MATTERS

 

Eligibility:

 

Iowa-based statewide, regional, or local agencies, OR emergency medical service programs (IDPH authorized medical care ambulance serve or non-transport service) are eligible to submit a bid application in accordance with this RFB.  Applicant is required to maintain and provide to the Department, upon application, a current and valid email account for electronic communications with the Department.
